Please check with the funeral home you are working with, as many funeral providers will complete some of these forms on your behalf.
The Canada Pension Plan provides several benefits to the survivors of a contributor. Below are the most common, with qualifications and downloadable forms.
To qualify for the death benefit, the deceased must have made contributions to the Canada Pension Plan (CPP) for at least: one-third of the calendar years in their contributory period for the base CPP, but no less than three calendar years, or 10 calendar years.
For deaths occurring on or after January 1, 2025, the death benefit includes a top-up. To qualify for the top-up amount, the deceased must qualify for the death benefit and: have never received a disability benefit, post-retirement disability benefit or retirement pension under the CPP or Quebec Pension Plan (QPP), and not have a surviving spouse or common-law partner who is eligible to receive a survivor’s pension.

Common-law partner:
According to CPP legislation, a common-law partner is a person of either sex who has lived with you in a conjugal relationship for at least one year. If you are a separated legal spouse and the deceased had no common-law partner, you may qualify for this benefit.
If you received a CPP credit split:
You are not eligible to receive a survivor’s pension if you are a separated legal spouse whose CPP credit split request was received and approved in January 2025 or later, for the same deceased contributor. However, you may still be eligible if you reunited with your separated legal spouse and lived together for 12 months or more immediately before their death.
If you previously remarried:
The rule was changed in 1987. If you previously lost a Canada Pension Plan survivor’s benefit because you remarried, contact Canada Pension Plan to find out if you are now eligible.
If you are widowed more than once:
Only one survivor’s pension — the larger — will be paid.
If you remarry:
Your pension will continue even if you remarry.

The Canada Pension Plan (CPP) children’s benefits provide monthly payments to the dependent children of disabled or deceased CPP contributors. There are two types:
A child can be paid a maximum of two children’s benefits.
Do you qualify?
The child must be either under age 18, or age 18 to 25 and in full-time or part-time attendance at a recognized school or university. The child must also be:
A child may be eligible if the parent or guardian is receiving a CPP disability benefit, or has died and met the CPP contributory requirements for a CPP death benefit. If the child is age 18 to 25, they must be in school to receive a children’s benefit.
Note: If the disabled contributor is over the age of 65 and the child stops attending school, the child’s eligibility to the disabled contributor’s child’s benefit ends.

Veterans and active service members have access to specific funeral and burial assistance programs.
The Last Post Fund provides funeral and burial assistance to eligible veterans. The current maximum rate payable toward funeral home costs is $7,376 + tax.

This form can be completed with a funeral benefit application or after an approval letter has been received. Use it to request funds in excess of the maximum funeral benefit rates, in extraordinary circumstances.

As of January 1, 2026, the WCB Alberta maximum for burial, cremation, and memorial expenses is $19,800.00.

Victims of violent crime can apply for assistance, supports, and services to help with emergency expenses and their recovery. The person(s) who paid for funeral expenses can be reimbursed for expenses up to $12,500.
